We are recruiting an SEN Transition Support Officer; to enable young people with learning difficulties & disabilities to achieve their full potential at crucial transition points. To make suitable, well-informed decisions about the next steps in education, training or employment:
Responsibility
Responsible for caseload management of children and young people 14 – 25 years attending mainstream school and post 16 provisions to coordinate multi-agency involvement across education, health and social care to facilitate positive outcomes across the transition between children's and adult services:
Qualification – Essential
· GCSEs (A-C)or equivalent to include Maths and English
· A specialist qualification in SEN
· NVQ4 in Advice and Guidance
· Advanced user – Microsoft Office, Word, Excel, Outlook, Social Media
Essential - Skills, Abilities & Experience
· Considerable experience in a senior role supporting young people with special
educational needs from year nine and within further education
· Strong knowledge of further educational pathways and support for exceptional
educational
needs young people
· Knowledge of local education, employment and training opportunities
· Working knowledge of the SEND Code of Practice and preparation for adulthood
the transition from year 9
· Working knowledge and experience of the statutory process for Education, Health and
Care assessments and plans
· Thorough understanding of partner agencies' current policies and procedures, e.g. Social
Care inc Early Help, Sensory Inclusion, Health, CAMHS etc.
· Knowledge of factors affecting young people and their transition to adulthood
· Experience in supporting young people to move towards positive destinations, including
employment
· Working with young people in a variety of settings
· Working and providing guidance and challenge to educational settings
· Ability to respond to deadlines and work within statutory timescales
· Strong team and partnership working
Essential Compliance Requirements
· Five Years of Reference
· Enhanced DBS is essential
· A pre-engagement screening is mandatory for this role
