Our local government clients in Merseyside require a Social Worker - Standard (Adults) to start immediately. The transition team works with young people from 14-25, the aim of work with 14-17 year olds is to plan for care act eligibility assessment (social care need assessment), ensure young person, family and other involved in their care are prepared for transition to adult services, that they have care planned and in place ahead of their 18th birthday and that following this their support is reviewed and support provided to ensure further smooth transition from education to community based placements and to settle young people into long term sustainable and appropriate accommodation (including sustaining support within family home).
Your key responsibilities will include:
To engage in social care needs assessments of young people, develop support packages, and review them, utilising Mental Capacity Act assessments where appropriate. They will initiate COP/DOL processes if required and work closely with colleagues in children's social work teams and leaving care teams, as well as with adult services, to ensure planning is in place for a smooth transition to adult services. They will work closely with health colleagues in a range of teams, including Learning disabilities, Mental Health (children's and adults), and ICB.
Qualification – Essential
- GCSE Grade (or equivalent) in English Language and Mathematics.
- A recognised degree in Social Work, DipSW or CQSW.
- Social Work England registered.
- Advanced Microsoft Office, Word, Excel, Outlook, Social Media and CRM.
Qualifications, Knowledge, and Experience Requirements - Criteria to be Evaluated at the Shortlisting Stage.
- Experience of social work in transition services, particularly completion of SCNA to establish care act eligibility, working closely with health and social care children's and adults' services and experience of undertaking mental capacity and best interest processes.
- Experience of using liquid logic systems, good time management, excellent engagement skills, and the ability to both Challenge when appropriate and ask questions when needed.
- Experience of COP/DOL processes, children's and adult services, and adult safeguarding processes, as well as knowledge of trauma-informed approaches, current developments in transitional safeguarding, and awareness of PBSS/active support models, would also be desirable.
