Our local government clients in Nottinghamshire are recruiting a Social Worker – Adult Care & Health > Community to deliver high-quality, effective social work services, responding promptly to the needs of adults and carers, and participating in assessments, care planning, and safeguarding.
Your key responsibilities will include:
Be responsible for identifying potential Reablement opportunities and providing access to those services as required. Monitor and review ongoing service provision, ensuring all parties involved are fully coordinated, with a focus on wellbeing and outcomes, and that all alternative solutions have been considered. Liaise and negotiate with local providers and support networks to deliver better outcomes. Provide professional, detailed information, advice and support to people and their careers, staff and colleagues, by building positive relationships and partnerships. To undertake safeguarding work in the role of Safeguarding Officer. Undertake the role of Approved Adult Mental Health Practitioner (AMHP), Best Interests Assessor (BIA), or Approved Mental Capacity Professional (ACMP) when trained and approved.
Qualification – Essential
- GCSE Grade (or equivalent) in English Language and Mathematics.
- Social Work qualification recognised by the Health and Care Professions Council (HCPC).
- Social Work England registered.
- Advanced Microsoft Office, Word, Excel, Outlook, Social Media and CRM.
At the Shortlisting Stage, we will evaluate candidates based on the following qualifications and Experience Requirements. These criteria are essential for the successful execution of the Social Worker – Adult Services role.
- Detailed knowledge of community care services within health or social care settings.
- Detailed knowledge of the legislation and policy in relation to adult community care services.
- Detailed knowledge of current adult social care and health policy drivers.
- Experience in carrying out comprehensive social work assessments using the Care Act.
- Experience of providing information, advice and signposting, to enable them to make informed decisions.
- Experience of working in partnership with people, their careers and service providers
- Demonstrable ability to monitor and review the impact of support plans against the agreed outcomes to ensure they are effective.
- Demonstrable ability to produce high-quality recordings, contact details, assessments, and support plans.
- Ability to prepare and present high-quality reports for use in legal cases, including the Court of Protection.
- An Understanding of and commitment to actively supporting and promoting equality within the workplace and the community, and equity of access to services for residents.
- Able to make judgements of need using clinical reasoning and record these accurately.
- Able to work collaboratively with service users, carers and a wide range of stakeholders at all levels and across a variety of organisations.
- Good demonstrable Understanding of the Care Act, and the Mental Capacity Act /DOLS.
- Good working knowledge of the London Multi Agency Adult Safeguarding policy and procedures and all related adults' legislation, including Making Safeguarding Personal.
- Knowledge of current best practice and integration initiatives across health and care areas.
- Knowledge and Understanding of the needs of Adults at Risk and those with long-term conditions and complex needs.
- Good Understanding of the issues relating to the delivery of assessment and support planning for people within a diverse community setting.
