We are seeking a Social Care Assessor who will be committed to working as part of a team, providing quality services that promote the principles of empowerment to support adults at risk, their careers, and advocates in achieving their outcomes. You will be required to use assessment, judgment, and decision-making skills with a focus on supporting the adult in managing risk and identifying the need for support and assistance.

Responsibility

To carry out high-quality holistic assessments, using strengths-based, person-centred, community-focused, and multi-agency interventions to bring about positive change through developing effective relationships and to secure the best outcomes for people and carers.

 Qualification – Essential

  • GCSE Grade (or equivalent) in English Language and Mathematics.
  • NVQ Level 3 in Health and Social Care (Adults) or equivalent experience
  • Advanced – Microsoft Office (Word, Excel, Outlook), social media and CRM.

Qualifications, Knowledge, and Experience Requirements - Criteria to be Evaluated at the Shortlisting Stage.

Successful candidates will have:

  • Knowledge of safeguarding procedures, current legislation and government initiatives.
  • Excellent communication skills, both verbal and written, with the ability to work with a diverse range of agencies and teams.
  • Good understanding of adult community care activity concerning statutory, voluntary or private social care sector.
  • Knowledge of the causes and effects of common disabling medical conditions.
  • Considerable experience working within a social care setting, working with people with disabilities or older people.
  • Able to manage a workload to agreed targets and able to use initiative and work without direct supervision.
  • Experience of working with vulnerable adults in the community, with knowledge of assessing need via assessment or review, or using assessment skills to meet the current need.
  • Ability to meet the requirements of the Learn to Care / Skills for Care / SCIE National Safeguarding Framework for safeguarding adults at level A, or the equivalent in any replacement for that framework
  • Knowledge of statutory requirements, including requirements in respect of carers, equality and anti-discrimination legislation, maintaining a safe working environment, data protection and confidentiality.
  • Experience in inter-agency collaboration practice.
  • Experience in undertaking safeguarding investigations.
  • Effective time management and planning skills to meet deadlines.
  • Ability to attend meetings outside of regular business hours.
