- Our client in Central London is recruiting a Supported Pathway Officer - To Assist residents in securing their next stage housing, providing resettlement support so that they successfully move into and sustain their new home. To provide appropriate floating support for six months and to ensure they are linked to necessary support services after that:
Responsibility
To provide assertive client-centred key-working, risk assessment and support planning to a caseload of approximately ten clients aimed at supporting them to maintain their current accommodation and link into services and meaningful activities which help them to improve their physical and mental well-being and life skills:
Qualification – Essential:
GCSE Grade A-C (or equivalent) in English Language and Mathematics
NVQ Business Administration level 3, BTEC or equivalent
Advanced – Microsoft Office, Word, Excel, Outlook, social media and CRM.
Essential Skills, Abilities & Experience:
· Knowledge of the impact of Adverse Child Experiences and the principles of Trauma
Informed Care when dealing with homeless people who have complex needs
· Knowledge of the Care Act and Housing legislation and the available support for single
homeless adults
· Knowledge of welfare benefits and other support that is available to residents of supported
accommodation
· Ability to support and motivate residents of supported accommodation
· Health and Safety issues about a supported housing setting
· Experience working with homeless people with complex support
needs
· Experience recovery-focused and harm-minimisation service provision for clients with
multiple and complex needs
· Experience in risk assessment, essential working and support planning
· Knowledge of case coordination in a multi-agency setting
· Ability to provide holistic client-centred assessments for Residents with complex support
needs
· Ability to communicate well with clients and colleagues
· Ability to plan and organise own workload
· Ability to record written/financial information accurately
· Ability to prepare simple reports and correspondence
Essential Compliance Requirements
· Five Years of Reference
· Enhanced DBS required
· A pre-engagement screening is mandatory for this role
