To work with individuals and families in an outcome-focused way using Strengths-Based Approaches:
 
Responsibilities
To manage a caseload of cases and complete Care Act assessments, reviews and update care and support plans. To collaborate with partner agents in health, SEN and the voluntary sector etc. The team promotes a hybrid working pattern; the successful candidate will have opportunities to work from the office as well as working from home:
 
Qualification – Essential
·         GCSEs (A-C) or equivalent to include Maths and English
·         Degree in Social Work or CQSA, Dip SW, CSS
·         Completed Care Act, MCA Dols and Human Rights Act  training, including Making
          Safeguarding Personal training
·         Social Worker (SWE registered)
·         Advanced user –Microsoft Office, Word, Excel, Outlook, Social Media
 
Essential - Skills, Abilities & Experience
·         To have a comprehensive working knowledge of legislation and how it impacts the role,
          including the Mental Capacity Act 2005, Best Interest, Deprivation of Liberty, Care Act
          2014, Mental Health Act 1995,  and the Human Rights Act 1998
·         Proven ability to work with customers with a range of needs and, at times, present with
          challenging or emotional behaviour
·         Previous experience of working with adults with learning disabilities
·         Exceptional ability to communicate effectively both orally & in writing to a wide range of
          people
·         Strong ability to organise own workload to meet targets & deadlines
·         Proven ability to work as part of a team and on own initiative
 
Essential Compliance Requirements
·         Five Years of Reference
·         Enhanced DBS
·         Social Worker (SWE registered)
·         A pre-engagement screening is mandatory for this role
