Description
We are looking for skilled and experienced social workers to join the Transforming Care team in the Adult Learning Disabilities area of Adult Social Care. The Transforming Care Team works with adults with learning disabilities and/or autism who are at risk of specialist inpatient admission.
Responsibility
The aim is to avoid admission through robust support planning and MDT collaboration and expedite discharge to the community for existing inpatients.
Qualifications
- GCSE Grade (or equivalent) in English Language and Mathematics
- Recognised Social Work Degree or other appropriate professional qualification.
- Social Work England (SWE) Number
- Advanced – Microsoft Office, Word, Excel, Outlook, social media, and CRM.

- Skills in undertaking strength-based assessments.
- Excellent communication skills with a wide range of people and mediums.
- Ability to undertake safeguarding enquiries.
- Ability to use the Mental Capacity Act (2005) to determine an adult’s capacity and to consider making best-interest decisions where appropriate.
- Good teamwork and collaborative working skills, working in partnership with colleagues and other professionals.
- Ability to listen and work with adults in a person-centred approach.
- Practical organisational skills and ability to set priorities.
- Ability to manage workload and make sound decisions even when under pressure.
- Ability to use the electronic client recording system and mobile working tools.
- Demonstrate a sound working knowledge of Adult Social Care legislation and practice.
- Knowledge of relevant current Social Care and Health legislation relating to adults and young people in transition, e.g., the Care Act (2014), Mental Capacity Act (2005), and Health & Social Care Act (2022).
- Working knowledge of local authority, health, public health, regulations and voluntary sector duties and responsibilities.
- Demonstrate sound knowledge and commitment to Equal Opportunities.
- Relevant experience working with adults in the community and/or hospital settings.
- Working across a range of agencies and understanding their roles.
