We are recruiting a Lead Practitioner for Front Door; to Take a lead in developing high-quality social care occupational therapy practice and the professional development of staff and students across Adult Social Care and Health through specialist support and advice:
Responsibility
Take responsibility for managing a small caseload of more complex cases. Provide consultation and advice to support social workers/ occupational therapists undertaking complex assessments and interventions, encouraging the development of high-quality standards and practice within a culture of continuous service improvement, value for money and safer practice.
Qualification – Essential
· GCSEs (A-C) or equivalent to include Maths and English
· Education to Degree level or equivalent and substantial experience
· Professional membership (e.g., HCPC)
· A qualification in leadership, management or business
· Advanced user –Microsoft Office, Word, Excel, PowerPoint, Outlook, social media:
Essential - Skills, Abilities & Experience
· Fully competent in the use of standard Microsoft Office products such as Outlook, Word,
Excel, PowerPoint
· Proven competency in handling confidential/sensitive personal information appropriately and
securely by Data Protection and Caldicott principles.
· Extensive knowledge, experience and understanding of delivering high-quality and cost-
effective services for service users.
· Proven in-depth experience in case management, handling complex cases in a highly skilled
and effective manner.
· Proven ability to apply an individualised, outcome-based approach to service delivery.
· Sound practical understanding of policies, processes and legislation necessary to deliver
complex services to service users and carers.
· Excellent written and verbal communication skills.
· Excellent case supervision skills and experience in effectively overseeing complex cases.
· Experience reviewing and evaluating results against quality standards, sharing with others
and taking decisive action to ensure that standards are delivered.
· Able to Demonstrate a record of high performance with a determination to provide a first-
class quality service with a history of achievement in successfully returning to a demanding
workload.
· Able to establish strong working relationships and networks with internal and external
partners.
· This 36-hour-a-week role offers hybrid working with a mixture of office and home working.
