We are recruiting a Team Manager within our Hospital's Integrated Discharge Team to lead and manage the team to ensure we are meeting statutory and locally agreed obligations, working to improve the efficiency and effectiveness of the Integrated Discharge Team:
Responsibility
To supervise and performance managing the team, advising on the assessment of needs and the management of risk, including Safeguarding enquiries:
Qualification – Essential
· GCSEs (A-C) or equivalent to include Maths and English
· CMI Level 5 Diploma in Management and Leadership relevant experience
· HCPC Registration
· Advanced user –Microsoft Office, Word, Excel, Outlook, Social Media:
Essential - Skills, Abilities & Experience
· Knowledge and experience in adult social care legislation, best practice, best value and
quality assurance principles
· Ability to take a lead role in promoting effective collaborative working with internal and
external stakeholders
· Ability to respond to enquiries and provide information and advice to Adults & Older
people and carers
· Experience in ensuring the safeguarding of adults and children at risk from abuse,
neglect or exploitation
· Ability to respond to concerns of adult safeguarding enquiries, assessments, and reviews
· Experience in client acute hospital discharge and dealing with complex situations whilst
working collaboratively with other professionals
· Ability to undertake all duties with due regard to the provisions of health and safety
regulations and legislation, Data protection/GDPR, the business's Equal Opportunities
and Customer Care policies
· Experience in close management of highly complex cases or cases where a high level of
unaddressed risks
· Ability to assist in ensuring that budgetary and other resource issues are considered as
part of the decision-making
· Experience in undertaking employee investigations and appeals in line with the business
policies and procedures
· Ability to implement and manage systems for the collection of performance information,
audit and case recordings
· Experience in supervising and performance management of a staff team
· Ability to work well under pressure and in situations that may become challenging
· Ability to work flexibly to meet competing needs and demands
· Ability to communicate effectively numerically, orally & in Writing to a Wide Range of
People
Essential Compliance Requirements
· Five Years of Reference
· DBS - Enhanced
· A pre-engagement screening is mandatory for this role
