We are recruiting a Support Worker within our adult learning difficulty to carry out evaluations and risk assessments and to liaise with MDT and families. To allow a smooth transition between early help and social care; arranging Family meetings and supporting families to improve outcomes for their children by taking on the role of the Lead Professional:
Responsibility
To provide intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes and supporting adolescents with complex emotional and mental health problems:
Qualification – Essential
· 3 GCSEs (A-C) or equivalent to include Maths and English
· NVQ Level 4 &/or relevant experience
· A full Driving Licence is a mandatory requirement
· Advanced user –Microsoft Office, Word, Excel, Outlook and social media:
Essential - Skills, Abilities & Experience
· Ability to assess risk and need through early help assessments on cases
· Experience in using de-escalation methods to reduce challenging behaviour from adults
and children, and young people in families
· Experience in supporting families and carers in developing practical parenting strategies
which enhances positive Parent/carer-child relationship
· Maintain social distancing wherever possible
· Ability to keep adequate records, plans, assessments, chronologies and associated
general notes on the electronic systems
· Ability to attend regular supervision sessions with the Consultant Social Worker providing
evidence of the outcome
· Ability to support Customers in a range of community-based activities and employment
opportunities that maximise choice and participation and, if required, work flexible hours
to provide the support identified
· Experience in visiting children, young people and families in their home environment to
provide targeted outcomes
· Ability to support the delivery of supervised contact to cases open to Early Help
· Ability to attend staff meetings, reviews, planning meetings and conferences and
contribute towards plans and objectives for children and young people
· Knowledge of different methods of family support and intervention
· Working knowledge of other relevant legislation, relevant national resources and relevant
experience in working directly with adolescents and other agencies to resolve conflict and
reduce risk
· Ability to think creatively, problem-solve and meet deadlines
· Ability to mentor others and apply good standards of practice
· Experience and knowledge of using Positive Behaviour Support (PBS)
· Ability to work well under pressure and in situations that may become challenging
· Ability to communicate effectively both orally & in writing to a wide range of people
Essential Compliance Requirements
· Five Years of Reference
· Enhanced DBS
· A pre-engagement screening is mandatory for this role
